Instructions to Book Your First Appointment:

1.Create Your Account
a) Go to TELUS CHR Connect.
b) Click “Don’t have an account? Sign up.”
c) Register with your email and a password.
d) Log in to your new account.

2.Connect to Our Clinic
a) When prompted, search for “Creekside Health Clinic.”
b) Select our clinic to connect.

3. Book Your Appointment
a) Choose a physician and schedule your New Patient Appointment.
b) Enter your provincial health number as the “Required Identification Field”:
• BC residents: Enter your PHN (usually begins with 9).
• Other provinces: Enter your provincial health number.
• Out of country: Select Private ID and enter any number to proceed.

4. Stay Updated
a) Select both email and text notifications to receive appointment updates.
b) Manage appointments anytime through the TELUS CHR app or website.
